Gold skulls are not lit for Gold & Souls Cache even though the game is reporting 711 of 711 skulls received. It is also letting me re-enter the quest...

I have the issue with Gold and Soul Cache. Not sure if I cleared it once, but the game still tells me it is not clear. I can get to the team selection screen and hit battle. It tries to load then takes me back to the team selection screen. I cannot re-run the dungeon, but the red indicator on the main screen tells me I have an available item and there is no timer set when I go to the Skull Quests list. Only happening with Gold and Soul Cache.

It has happened to me if the game crashes during the battle. When I restarted the game I sometimes recieved the rewards sometimes not. It should go away after reset.

Axes grind and maces clash as wounded fighters fall to the ground
Severed limbs and fatal wounding bleeding corpses lay all around
