Greetings,
I have completed A matter of life and death on hard many times under 300 seconds but never received the skull. I even set a timer multiple times. Has this bug been reported or fix of workaround been mentioned?

Yes, this has been reported. Workaround is complete it much faster, or complete it with 1xspeed.
It appears to me that playing with 2xspeed will cause this timer to run faster too.

Yes, happened the same to me. I got the skull finishing in 200 seconds approx.
